Client phone as suffering diahorea in the last few days, She went away on holidays and that's went started, (Friday) settle by Monday but Tuesday was a an injection day and symptoms returned, advice given on managing symptoms including electrolites and loperamide, client is going on holidays when next dose is due, agreed to missed that dose and restart after the holidays
client moving from borders aesthetics, medical history reviewed , no contraindications to treatment, currently on Mounjaro 7.5mg, current dose works well, appetite reduced however wight loss slow down

current weight 91.6kg (-1.4kg in the last 4 weeks and -28.4kg since start)
tolerating medication well, slight constipation, manageable advice given, rash at injection site, client changed needles to 6mm and that seems to be working well in managing local reaction, prescription for 7.5mg pen issued
