following reports of feeling unwell, two attempts to contact Dean to check how is he feeling, Friday 14/08/26 client unwell to talk due to sickness. Client mentioned will call back but no contact. Follow up with the message, Dean reply he is feeling well and will get in touch if anything changes. Follow up call booked

Deans Mum contacted me as she was concerned how Dean is feeling in the last to weeks. Spoke to Dean on the phone, for the last two weeks he is experiencing severe fatigue with frequent bowel movements (not loose stools). Discussed diet- poor diet at the moment but he felt rubbish to put effort into healthy eating. No other contributing factors. Taking into consideration duration of treatment and the fact it is affecting Deans everyday life advice to make appointment with the GP, follow up call booked in a few days. Dean expresses permission to share any information with his mum Susan Armstrong
